HOW TO ENTER/ WINNER SELECTION: HOW TO ENTER/ WINNER SELECTION Listen carefully to KQCH-FM Mondays through Friday between July 14th through August 1st, from 7-10pm CST during for the sound of the prompt to call KQCH-FM. Call in number is 402-938-9400. To become one of fifteen (15) on-air semi-finalist qualifiers eligible to be entered into the drawing for a chance to win the Grand Prize, you must be the 9th caller received following the prompt as determined by KQCH-FM. The prompt will be randomly made at various times throughout the hour for a total of one daily prize winner per day. The 9th caller must give Matt the Triple DEWmocracy Playlist. If the 9th caller correctly lists the Triple DEWmocracy Playlist, they will be a daily prize winner. If caller #9 does not correctly list the Triple DEWmocracy Playlist, the next caller who does so will be the daily prize winner. Limit one Daily Prize per person throughout the sweepstakes period. Odds of winning depend upon the number of call-ins received at each prompt. All Daily Prize winners will be automatically entered into the Grand Prize, First Prize, and Second Prize drawings. Daily prize winner will receive a Third Place or Fourth Place prize as their daily prize.
To enter at Anderson Food Shops, hand print your name, complete street address and phone number on an official entry form available from KQCH-FM at radio remote events or on a piece of 3” x 5” inch paper. Drop your completed entry in the KQCH-FM box at each radio remote event. Limit one entry per person, per radio remote. No photocopied or mechanically reproduced entries accepted. No responsibility is assumed by KQCH-FM or Pepsi Bottling Group for lost, late, illegible or misdirected entries. All entries must be received no later than August 3, 2008. Random winners will be selected to receive a Fifth Place Prize.
One (1) Grand Prize, Two (2) First Prize, Two (2) Second Prize winners will be selected in a random drawing from among all eligible on-air and in-store qualifiers received throughout the sweepstakes period. Grand Prize Drawing will be conducted on-air by KQCH-FM on August 3, 2008. The name and address of potential Grand Prize winner may be announced on-air following drawing. KQCH-FM reserves the right to notify and/or contact the potential winner by phone and to broadcast all or a portion of the notification phone call on air in either a live or pre-recorded format as determined by KQCH-FM. Odds of winning Grand Prize depend upon the number of qualifying entries received.
1. PRIZES:
GRAND PRIZE: One (1) grand prizes will be awarded each consisting of a Laptop (approximate retail value $800)
FIRST PRIZES: Two (2) first prizes will be awarded each consisting of a multi-function backpack with built in speakers and microphone (approximate retail value $150)
SECOND PRIZE: Two (2) second prizes will be awarded each consisting of a $100 gift card from Amazon.com (retail value $100)
THIRD PRIZE: Fifteen (15) third prizes will be awarded each consisting of a Mountain Dew Voltage branded USB Flash Drives. (approximate retail value $5)
FOURTH PRIZE: Twenty-five (25) fourth prizes will be awarded each consisting of a Mountain Dew Revolution branded Scanster Radio (approximate retail value $5)
FIFTH PRIZE: Thirty (30) fifth prizes will be awarded each consisting of a Mountain Dew Supernova branded Flash Light Keychain. (approximate retail value $5)
3. ELIGIBILITY: Sweepstakes open only to legal residents of the U.S. residing in the states of Nebraska and Iowa. Employees of Journal Broadcast Group, Pepsi Bottling Group, Pepsi-Cola Company, its bottlers, their respective parents, subsidiaries, affiliates, divisions, distributors, suppliers and advertising, promotional or judging agencies and immediate family members of such employees (parent, child, sibling, spouse) and persons living in the same household of such employees (whether related or not) are not eligible to win. If potential prize winner is a minor, prize may be awarded to the minor's parent or legal guardian who must sign all prize acceptance documents assume all responsibilities of prize winner.
4. GENERAL: No substitution of prize is offered, no transfer of prize to a third party is permitted and non-cash prizes may not be redeemed for cash value, except as determined by KQCH-FM and Pepsi Bottling Group in its sole discretion. Void wherever prohibited or restricted by law. Prize winner will be solely responsible for paying all applicable federal, state and local taxes on prize; if applicable, winner may receive an IRS Form 1099 for the retail value of the prize. No groups, clubs or organizations may enter or win prizes. All entrants, as a condition of entry, agree to be bound by these rules and the decisions of KQCH-FM and Pepsi Bottling Group, which shall be final and binding.
5. WINNER NOTIFICATION: All potential winner(s) will be notified by phone at the phone number provided in entry. Major prize winners will be required to sign and return to KQCH-FM an Affidavit of Eligibility/ Liability Release with (where lawful) a publicity release within five days of first attempt at notification. If any required documents are not returned within the required number of days, or if mail is returned as undeliverable, without a forwarding address, then prize may be forfeited and an alternate winner may be selected. Allow 4-6 weeks for delivery of Grand Prize by mail following completion of notification process. Prizes will only be delivered to addresses within the United States. Winner must personally pick-up First, Second, Third, Fourth and Fifth Prizes at KQCH-FM offices within 30 days and present identification for prize to be released. All travel and transportation costs, if any, associated with collecting the prize, are winner's responsibility.
6. RELEASE OF LIABILITY. All entrants, as a condition of participation, and all winners, by acceptance of a prize, agree to release KQCH-FM, Pepsi Bottling Group, Pepsi-Cola Company, its bottlers, each of their respective parents, subsidiaries, divisions, affiliates, suppliers, divisions, distributors and advertising, promotional or judging agencies from any and all liability, claims or actions of any kind whatsoever for injuries, damages or losses to persons and property which may be sustained in connection with the receipt, ownership or use of the prize or while preparing for, participating in, and/or traveling to and from any prize-related activity or any typographical or other error in the printing of these Official Rules or the offering or announcement of any prize. Neither KQCH-FM, Pepsi Bottling Group nor Pepsi-Cola Company make any representations nor warranties of any kind concerning the appearance, performance or safety of any prize awarded. Upon request, winners must consent (and confirm such consent in writing) to the use of their name, voice, picture or other likeness without compensation for promotional purposes as determined by KQCH-FM and Pepsi Bottling Group except for residents in TN and where prohibited by law.
