**Channel 94.1 Diaper Drive Volunteer Waiver & Release of Liability**
Event Dates: December 12–13, 2025
VOLUNTEER AGREEMENT
By participating as a volunteer in the Channel 94.1 Diaper Drive, I hereby voluntarily agree to the following terms:
1. Release of Liability
I release, indemnify, and hold harmless SummitMedia, Open Door Mission, Hy-Vee, Werner Enterprises, Woodhouse Auto Family, their employees, representatives, partners, volunteers, and successors from any and all claims, demands, suits, actions, judgments, damages, losses, costs, or expenses arising out of any injury to me or my property that may occur during or as a result of my participation in this volunteer activity.
2. Assumption of Risk
I acknowledge that I am fully aware of all risks associated with volunteer activities and affirm that I have no known medical condition or limitation that would prevent or restrict my safe participation.
3. Photo/Media Release
I grant permission for project partners, event organizers, and the media to photograph or record me during volunteer activities. I authorize these images to be used in printed materials, social media, websites, broadcasts, and other promotional or informational outlets without compensation.
4. Acknowledgment
By signing below, I confirm that I have carefully read, fully understand, and agree to the terms of this Volunteer Agreement.
